You can breastfeed without a special nursing shirt, but it's far, far easier with one. Whoever tries it, no longer bothers breastfeeding in regular clothes, during which either the entire chest or belly must be exposed. Breastfeeding in public in "non-breastfeeding" clothes also usually attracts unwanted attention. Breastfeed in a Katherine T-shirt and anything that is exposed will be covered by the feeding baby's head. Plus, you and baby won't be annoyed by any fabric of a rolled-up or bunched-up shirt that you have to hold on to.
